Become A Memeber

Becoming a member of the Clearwater Ostomy Support Group is as easy as 1 - 2 - 3.

1.  Click on application , print it and fill it out.

2.  Enclose a donation for $18.00.

     (Donation is tax-deductable)

3.  Mail the application and donation to the address

     on the form.

Its that easy!  And once you are a member of the Clearwater Ostomy Support Group you, too, can enjoy all the benefits that a membership provides.
